OSHA's Most Cited Manufacturing Violations in Material Handling

OSHA publishes its top ten most cited standards at the end of every fiscal year. The list barely changes. The same categories show up because the same gaps persist.

For fiscal year 2025, fall protection led with 5,914 citations. But the top ten is dominated by construction standards. For manufacturing plants, the citations that matter are the ones tied to material handling, lifting, and overhead equipment.

Overhead Cranes — 1910.179

The standard most directly relevant to material handling operations. Crane citations consistently fall into three areas:

Missing inspection documentation. The detailed breakdown is in our 1910.179 article, but the short version: without dated, signed inspection records, the citation is essentially automatic.

Inoperative limit switches. The upper limit switch is the most common crane-specific citation. Inspectors ask the operator to demonstrate it. If it doesn't trip, that's a serious violation.

Wire rope condition. Broken wires exceeding removal criteria, diameter reduction beyond manufacturer specs, kinking, or bird-caging. Inspectors carry calipers and know the measurements.

Powered Industrial Trucks — 1910.178

Forklifts interact with every material handling operation. The most common citations: untrained operators, expired certifications (re-evaluation is required every three years), and missing documentation of training completion.

The second category is pedestrian safety — undefined travel lanes, missing mirrors at blind intersections, no audible warning devices. Facilities where forklifts operate in the same aisles as crane-served workstations face compounded exposure.

One pattern that reduces this exposure: overhead crane systems and rail-mounted delivery eliminate fork truck traffic in production areas entirely. Fewer forklift movements in the work zone means fewer pedestrian interaction points.

Lockout/Tagout — 1910.147

LOTO applies to any equipment with stored energy — including overhead cranes, hoists, powered conveyors, and lift tables. The gaps that generate citations:

No written energy control procedure for the specific piece of equipment. Generic procedures don't satisfy the standard. Each machine needs its own documented procedure identifying all energy sources and isolation points.

Annual periodic inspections not conducted. Every energy control procedure requires annual review by an authorized employee who isn't the one who normally performs it. That review requires documentation. Most plants either skip this or do it without a paper trail.

Fall Protection — 1910.28

Fall protection in manufacturing shows up when workers access crane runways, mezzanines, loading docks, or elevated platforms for maintenance. Any access above four feet requires either guardrails, a personal fall arrest system, or a travel restraint system.

Fixed rail fall protection along crane runways is a one-time installation that eliminates the citation risk and removes the recurring cost of inspecting, fitting, and training on individual harness systems.

Cumulative Penalty Exposure

Serious violations carry penalties up to $16,550 per instance. Willful or repeat violations: up to $165,514. A single inspection that finds a missing crane inspection program, an inoperative limit switch, and no LOTO procedure for the hoist — that's three serious violations, potentially $49,650.

What follows a citation is often more costly than the fine itself. Increased inspection frequency means the probability of a follow-up visit within 12-18 months is high.
